1. A cover letter signed by the organization's Executive Director or Board President
   
   
2. One copy of the cover summary:

3. One copy of a brief proposal (5-6 pages excluding appendices). Proposals must include the following information, keeping in mind the funding criteria:
   
   
   • Applicant Organization (1 page): The Organization's mission and brief history; major programs and geographic scope of services; organizational structure and number of staff.
     
     
   • Proposed Use of Funds (2-3 pages): The project or purpose for which grant funding is sought; the target population and community needs to be addressed; the goals (qualitative and quantitative) to be achieved; and specific timetable of activities for attaining stated goals.
     
     
   • Evaluation Process (1 page): The criteria and process the organization will use to determine the project's success.
     
     
   • Project Budget (1 page): Please use the MSCF Budget Sheet to provide an itemized project budget showing:
     
     
     • all expenses (e.g. salaries, professional services, equipment, printing)
     • an indication of which project expenses will be covered by MSCF's funds
     • all other funding sources from whom funds for this project have been or will be requested and amounts of each source (in-hand and anticipated)
   
   
4. Appendices:
   • Current organizational operating budget
   • List of board of directors
   • IRS 501(c)(3) determination letter
   • Latest audited financial statement or the equivalent, as required by law
     
     
   
   
5. Please simply clip all documents together and refrain from using staples or binders.
